COVID-19 Acknowledgement of Risk and Consent Form.
Great Hill Track Club follows both the CDC’s guidelines for guarding against COVID-19 and New York State guidelines regarding travel and quarantine rules for those entering New York State. CDC guidelines can be found at:

The travel guidelines require all New Yorkers, as well as those visiting from out-of-state, to take personal responsibility for compliance in the best interest of public health and safety. It is the responsibility of every runner to stay-up-to-date and comply with New York State travel guidelines as these are always subject to change.
Occasionally, a small group of runners who are familiar with one another and mutually comfortable with pulling their masks down will do so during, for instance, a 400-meter pickup, and we don’t object to that. This method is currently employed at New York high school races, at NYC Runs events, and elsewhere: masks on before running, pulled down during the race, and then back on at the finish.
Please stay home if you exhibit any of the following symptoms: cough, fever (over 100), new shortness of breath, difficulty breathing, chills, runny nose/ stuffy nose (not related to seasonal allergies), sore throat, diarrhea, vomiting, loss of smell, loss of taste, odd bruising or discoloration of your toes, unexplained fatigue/muscle aches, rash, or headache (plus one other symptom noted above).
If you test positive for COVID within 2 weeks of attending a workout, please email greathilltc@gmail.com as soon as possible.
Please stay home if you have been exposed to someone infected with COVID-19 within the last 2 weeks.
Only team members are allowed at workouts.
Please refrain from spitting and shooting snot rockets.
Please comply with the New York State travel guidelines (link provided below).
If you see team activity that concerns you, please tell a coach.

Further information on the updated New York State travel guidelines:
Beginning November 4, 2020, new guidelines allow travelers to New York to “test out” of the mandatory 14-day quarantine. Travelers from states that are contiguous with New York are exempt from the travel advisory; however covered travelers must continue to fill out
the Traveler Health Form. Essential workers will continue to be exempt as well. The guidance also applies to international travelers coming from any CDC Level 2 or Level
3 Health Notice country.
For any traveler to New York State from a noncontiguous state, US territory or CDC level 2 or level 3 country, the new guidelines for travelers to “test-out” of the mandatory 14-day quarantine are below:
For travelers who were out-of-state for more than 24 hours:
Travelers must obtain a test within three days of departure, prior to arrival in
New York.
The traveler must, upon arrival in New York, quarantine for three days.
On day 4 of their quarantine, the traveler must obtain another COVID test. If
both tests comes back negative, the traveler may exit quarantine early upon receipt of the second negative diagnostic test.
For travelers who were out-of-state for less than 24 hours:
The traveler does not need a test prior to their departure from the other state, and does not need to quarantine upon arrival in New York State.
However, the traveler must fill out our traveler form upon entry into New York State, and take a COVID diagnostic test 4 days after their arrival in New York.
Local health departments will validate tests, if necessary, and if a test comes back positive, will issue isolation orders and initiate contact tracing. The local health department must make contact with the state the traveler came from, to ensure contact tracing proceeds there as well. All travelers must continue to fill out our traveler form upon arrival into New York State to contribute to New York State’s robust contact tracing program.
